Elizabeth D. Burnette, MSN, APRN, FNP-BC
Elizabeth Burnette, better known at “Beth” was born and raised in Nash County. She describes herself as, “A Country Girl that lives to Serve her Patients”
Beth has had an extensive and comprehensive medical career where she plans to continue with Hopper-Burnette Internal Medicine for the rest of her calling. Beth has been blessed to serve patients as a SOLE PROVIDER working independently in the fields of Family Medicine, Geriatrics, General Practice, Urgent Care, Trauma, Rural Health and ER. She is confident that she can care for you and your families’ needs as a primary care provider.
Before moving back to North Carolina to be close to family, Beth lived in Montana for approximately 20 years. As a FNP in Montana, she was allowed full practice Authority and practiced independently throughout the frontier regions of the state. She has worked in rural healthcare clinics and critical access hospitals as a Primary Care Provider, ER NP and Hospitalist provider, caring for those with simple medical needs to those with life threatening illness or injuries. Her great love has been working and living on the Crow/Northern Cheyenne Reservation delivering care to a very underserved community of Native Americans and ranchers.
Her esteemed career has spanned over 30 years and has included Geriatrics, Family Primary Care Specialist. Open Heart and Neurosurgery ICU, Open Heart and Neurosurgery ICU, Trauma Specialist, ER Specialist, Pediatrics, Home Health, Medical Surgical ICU Flight Nursing across Montana, Manager and Director of Nursing positions, Nursing Home Administrator, Hospitalist, and Urgent Care Specialist. She is board certified to care for patients of the entire life spectrum—birth to geriatrics. Beth graduated from Frontier Nursing University in Hyden, Kentucky with a Master’s Degree in Family Practice, also as an Honors Graduate and a recipient of multiple scholarships and honors awards. She has worked toward her Doctorate with the prestigious distinction of Magna Cum Laud, and with only two more classes to complete is taking a hiatus to enjoy life and help care for her elderly mother.
An avid horsewoman and lover of dogs, Beth now resides in Wilson County and plans to make this her “forever home.” With 2 horses and 3 rescue dogs, life on the farm is busy. Any spare time is spent out of doors, fishing, kayaking, hiking, or beach combing. Rainy days you will find her knitting, crocheting, sewing, and weaving with antique looms.

Axel L. Mercado, MPH, PA-C
Mr. Axel Mercado was born in Ponce, P.R, raised in New York City and relocated from Miami, Florida. He likes to be addressed by his first name “Axel” and describes himself as a patient advocate. Axel is Bi-lingual in English and Spanish. His goal is to help our patients achieve their best outcomes and health possible through an honest, open patient-provider relationship.
Axel is a veteran of the U.S Army, Army Reserve and Florida National Guard. While serving his country and state, he worked as a Medical Specialist which motivated him to become a Physician Assistant (PA) after working alongside PA’s.
He graduated in 1996 from Nova-SouthEastern University in Davie, Florida where he obtained a Bachelor Degree in PA Studies and a Masters Degree in Public Health. The programs voted him the Class President for the PA and Masters class. When he graduated, he was awarded the “Universities Chancellor's Award” and selected as the “Student Government Leader of the Year”.
Axel comes to “Forest Hills Immediate Care” with over 25 years of experience treating patients. His medical knowledge is diverse. It includes Primary Care, Urgent Care, Admissions Hosptialist and In-Patient Hospital Internal Medicine care. While in Miami, he was instrumental in getting its first PA program where he was the Academic Coordinator and an Associate Professor for the PA Program.
Prior to moving to North Carolina to practice, he was Specializing in Wound Management where he treated patients throughout the continuum of healthcare. He managed patients in their homes, out-patient clinics, Wound Care Centers, Nursing Homes, Skilled Care Facilities, Long-Term Acute Care and Hospital in-patient. Axel treated patients with pressure injuries, vascular and arterial ulcers, diabetic ulcers and complicated surgical wounds.
When not devoting his time to patients, you can find Axel hiking, mountain biking and traveling the world which is why he is part owner in a travel agency - Odyssean Travel.
